Log Number

A unique number assigned to an incident that links all phases of the notification process or administrative investigation and disciplinary recommendation, grievance process, arbitration, Police Board proceeding, and appeals.

Log Numbers are assigned to every incident involving a CPD member that is reported to the CPD or COPA and subject to investigation. In cases of an investigation, a Log Number is converted and classified as a “Complaint Register” (CR) number. Log Number investigations also may subsequently be classified as an “EO” (an extraordinary incident, e.g., a death in custody, a suicide in custody, or an attempted suicide in custody); an “Info” (an incident that has not been converted into a Compliant Register number due to the reporting party’s failure to sign a sworn affidavit); a “Notification” (an incident where a sworn CPD member discharges a firearm at a person and the person is not injured or killed; a weapon discharge incident involving the destruction of an animal; an Oleoresin Capsicum (OC) discharge; the field deployment of a Taser; the use of a chemical-dispensing, smoke dispensing, or distraction device; or another miscellaneous incident); or a “U” (an incident where a sworn CPD member discharges a firearm and another person is injured or killed, or where a sworn CPD member suffers a self-inflicted gunshot wound).
